CMCHIS requires Diagnostic Services as per � DC Procedure List (please follow link) in the districts of Ariyalur, Coimbatore, Cudallore, Dharmapuri, Dindigul, Kanyakumari, Karur, Krishnagiri, Nagapattanam, Namakkal, Nilgiris, Perumballur, Ramanthapuram, Sivaganga, Thiruvannamalai, Thiruvarur, Tiruppur and Villupuram.

Applications are invited from Diagnostic Centers from above districts to be selected for empanelment as a Network Diagnostic Centers under the Chief Minister�s Comprehensive Health Insurance Scheme by the Empanelment & Disciplinary Committee. The Application Form can be downloaded from the link down below. The list of such Diagnostic Procedures also can be downloaded from the link �DC Procedures� below. Interested Diagnostic Centers may submit applications by e-mail to [email protected] with required particulars and most competitive tariff rates. The applications must reach on any date till midnight of 25th February 2012. Decision to alter, reduce or extend the above dates for submission of applications as may be found expedient may be taken by Empanelment Discipliary Committee.
